If you are eligible for a Home Care Package and would like professional support, ask us how we can support you to stay independent and in your own home, as well as meeting your healthcare goals.
HOME CARE PACKAGES
To access the Home Care Packages Program you firstly need to be assessed by an Aged Care Assessment Team https://www.myagedcare.gov.au and be approved for a government subsidised Home Care Package https://www.myagedcare.gov.au/help-at-home/home-care-packages
